Feedback Forms
Collect responses with a drag-and-drop form builder — 20+ input types, multi-step layouts, conditional logic, and built-in analytics, with every response browsable and exportable.
Overview
A feedback form is a workspace item built in the form builder and shared with respondents through a link. Responses land in the workspace, where you can browse them one by one, see aggregate analytics per question, and export everything.
Forms can be a single page or a multi-step flow — add page breaks wherever you want a step boundary, and respondents get a progress indicator as they move through.
The builder
The builder is a drag-and-drop canvas: pick inputs from the library, drop them in order, and configure each one in the inspector — label, help text, required flag, and per-type options like choices or scale bounds.
Form-level settings cover the title and description, visibility, availability window, layout, the message shown after submitting, and an optional redirect URL.
Before sharing, the share dialog runs a pre-flight check — a name, at least one input, and no validation errors — so a broken form never goes out.

Who can open a form, and when, is controlled per form:
Conditional logic
A Professional feature
Logic rules show, hide, or require inputs based on earlier answers. Each rule is a group of conditions combined with “all” or “any” — for example, show a follow-up only when the rating is below 3, or make a comment field required when “Other” is selected.
Operators adapt to the input type: text inputs offer contains and equals checks, numeric and scale inputs compare values, and choice inputs match selected options. Rules can only reference inputs that appear earlier in the form.
Page breaks accept visibility rules too, so entire pages can be skipped — the form auto-advances when a page ends up empty.
Flow view
The flow view draws your form as a dependency graph — every rule becomes a color-coded edge between inputs, so complex branching stays readable. You can inspect and edit rules directly from the graph.
Responses & analytics
Responses appear in the workspace as they arrive. Open any response to see the full submission, or use the analytics dashboard for per-question aggregates like choice distributions and rating averages.
Everything exports to CSV, Excel, JSON, or PDF — the Analytics & Exports guide covers the dashboards and every export option in detail. Analytics and exports are included on every plan.
